Job Summary

We are seeking an experienced Advanced Nurse Practitioner to join our Primary Care Network in North Watford. As a key member of our team, you will be responsible for delivering high-quality patient care and contributing to the development of our health promotion and wellbeing programmes.

Main Responsibilities

  • Develop and implement health promotion and wellbeing programmes to improve patient outcomes.
  • Manage patients with acute and chronic medical conditions, providing expert advice and care.
  • Implement and evaluate individualised treatment plans for chronic disease patients.
  • Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on their health.
  • Provide advanced nursing care to patients, adhering to clinical guidelines and evidence-based practice.
  • Provide wound care, including ulcer and Doppler assessments.
  • Deliver specialist clinics, such as ear syringing and contraception services.
  • Collect and process pathological specimens, as required.
  • Provide travel medicine services and request pathology services as necessary.
  • Process and interpret pathology and other test results, ensuring accurate diagnosis and treatment.
  • Deliver chronic disease clinics, referring patients to secondary or specialist care as needed.

Person Specification

Essential Qualifications

  • Registered Nurse with the Nursing and Midwifery Council.
  • Qualified Independent Nurse Prescriber on the NMC register.
  • Masters degree required for qualification post December 2020 (refer to RCN Credentialing for Advanced Level of Nursing Practice).
  • Minor illness qualification.

Essential Experience

  • Venepuncture, ECGs, new patient medicals, chaperone procedure, diabetes, hypertension, CHD, women's health.
  • Experience of working as a practice nurse or community nurse.
  • Wound care and removal of sutures and staples.
