About the Role:
We are seeking a highly skilled and dedicated Social Worker to join our Disabled Children's Team within the Care Outcomes Service. As a key member of our team, you will work closely with children, young people, and families in the Bath and North East Somerset area, providing tailored support and services to meet their unique needs.
Key Responsibilities:
- Manage a caseload of children and families with complex needs, providing individualized support and interventions to promote positive outcomes.
- Work collaboratively with a multidisciplinary team, including Team Managers, Deputy Team Manager, Senior Practitioner, qualified professional Social Workers, Family Support Practitioners, Occupational Therapists, and Administrative staff.
- Develop and implement outcome-focused plans, analyzing information and formulating effective strategies to support children and young people.
- Provide professional assessments and reports, as well as presenting evidence verbally to stakeholders.
- Contribute to service development and improvement, sharing knowledge and best practices with colleagues to enhance the quality of services provided.

Requirements:
