Job Summary
We are seeking a dedicated and experienced support worker to join our team at CJP Outreach Services. As a Complex Support Specialist, you will play a vital role in supporting a young person with complex needs to live independently in the community.
Key Responsibilities
- Provide high-quality support to enable the individual to achieve their goals and live independently
- Work collaboratively with the individual, their family, and other professionals to develop and implement personalized support plans
- Assist with daily living tasks, including personal care, meal preparation, and household management
- Support the individual to develop and maintain social connections and relationships
- Participate in ongoing training and professional development to enhance your skills and knowledge
Requirements
- Experience working with individuals with complex needs, including PTSD, Autism, and FND Seizures
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information
- Valid DBS check
