Job Overview
This is an exciting opportunity for experienced Registered Mental Health Nurses looking to join the community mental health services in a dynamic and innovative environment. You will have excellent communication skills, be self-motivated, proactive, and committed to the promotion of high-quality service delivery.
The successful candidate will have the skills to deliver holistic, safe, and effective care to patients within the community, holding a complex caseload of individuals with severe and enduring mental health issues. You will work collaboratively to assess needs and create care and treatment plans to meet identified needs and risks.
You will be required to work independently and liaise with various agencies, third sector organizations, individual carers, and family members. This role will involve working within the Community Mental Health Team in Denbighshire, with potential opportunities to work in other teams and inpatient settings.
You will have exceptional communication skills and the ability to build and maintain effective working relationships, demonstrating a 'can-do' attitude.
Main Duties of the Job
The post holder will undertake Care Coordination of service users with complex mental health needs, facilitating the promotion and maintenance of a high standard of community psychiatric care. This may involve working in a clinic-led system for some service users.
The post holder will be expected to work in a recovery-focused way, facilitating user and carer involvement.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
