About the Role

We are seeking a skilled Clinical Pharmacist to join our team at Manor Medical Practice. As a Clinical Pharmacist, you will play a vital role in ensuring the rational and effective use of medication, working closely with healthcare professionals to provide excellent patient care.

Main Responsibilities

  • Provide structured medication reviews to optimize patient outcomes
  • Conduct acute medicines request reviews to ensure safe and effective treatment
  • Process hospital discharge/clinic letters to ensure seamless care transitions
  • Offer support and advice to patients with self-limiting conditions, promoting independence and empowerment
  • Collaborate with practices to achieve Quality and Outcomes Framework (QOF) targets
  • Contribute to the improvement of health outcomes in long-term conditions as part of the multidisciplinary Primary Care Team
  • Perform other general tasks as required to support the team

Requirements

  • Independent Prescriber Status (essential)
  • Previous GP Practice experience (essential)
  • Minimum of 3 years Post Graduate experience
  • Excellent clinical knowledge and commitment to ongoing learning and development
  • Strong clinical leadership, communication, and interpersonal skills
  • Proficient in SystmOne
