About the Role

We are seeking a compassionate and empathetic Support Worker to join our team at Harmony House. As a key member of our team, you will be responsible for providing person-centered support to individuals with learning disabilities and mental health diagnoses. Your role will involve implementing support plans, promoting independence, and ensuring the well-being of our service users.

Key Responsibilities
  • Implement person-centered support plans to promote independence and well-being
  • Develop flexible and engaging activity plans to promote autonomy and personal growth
  • Support individuals in maintaining healthy lifestyles and managing their health needs
  • Facilitate effective communication and maintain detailed, person-centered records
  • Oversee medication processes and support individuals in self-administration
  • Maintain a safe, clean-living environment and adhere to health and safety protocols
