The successful candidate will provide high-quality nursing care to patients in their own homes, working closely with the multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.

The ideal candidate will have a strong background in acute medicine, emergency care, or community nursing, with excellent leadership and communication skills. They will be able to work independently and as part of a team, with a strong focus on patient-centered care and community engagement.

Key Responsibilities:

  • Provide comprehensive nursing assessments and interventions in patients' own homes
  • Develop and implement individualized care plans to meet patients' needs
  • Collaborate with the multidisciplinary team to ensure seamless care transitions and optimal patient outcomes
  • Conduct clinical triage and make decisions regarding patient care
  • Prescribe medications and treatments as needed
