Job Title: Community Homecare Nurse

This is an exciting opportunity to join our team as a Community Homecare Nurse in the Chalfont's and Gerrards Cross area. We are seeking a highly motivated and effective team player to provide a high-quality nursing service within the District Nursing field.

Main Responsibilities:
  • Work within a locality team to meet the needs of complex patients in the community and avoid admissions to acute settings.
  • Provide medication administration, end-of-life care, and manage deteriorating patients, including complex and simple wound care.
  • Utilize advanced assessment skills and clinical nursing skills to nurse patients with various conditions in a holistic manner.
  • Work in close partnership with social care and other primary healthcare professionals, such as GPs, Community Therapies, and specialized services.

Person Specification:
  • Registered Nurse or student nurse awaiting confirmation of NMC registration.
  • Committed to own professional development to enhance practice.
  • Post-registration education and extended skills, such as knowledge of the NMC Code of Professional Conduct and the Data Protection Act.
Desirable Qualifications and Experience:
  • Demonstrate experience in relevant clinical areas, such as Care of Elderly, Community, Orthopaedics, Neurology, Respiratory, Palliative Care, and Long-term conditions.
Contract and Working Pattern:

Permanent, full-time, part-time, job share, or flexible working arrangements.
