About the Role

We are seeking a highly motivated and experienced Pharmacist to lead our pharmacy team in a challenging and rewarding role.

The successful candidate will have a strong background in clinical pharmacy, with experience working in primary care or a similar setting. They will be responsible for leading the team in providing high-quality patient care, managing medication reviews, and contributing to the development of our pharmacy services.

Key Responsibilities:

  • Lead the pharmacy team in delivering high-quality patient care and improving health outcomes.
  • Manage medication reviews, including structured medication reviews for patients with complex polypharmacy and multi-morbidities.
  • Contribute to the development of our pharmacy services, including the annual Prescribing Quality Scheme (PQS) targets.
  • Support colleagues in all areas of medicines management and optimisation, including medicines reconciliation and clinical audits.
  • Promote efficient and safe repeat prescribing processes with a focus on patient safety, reducing overprescribing and medicines waste.
  • Liaise frequently with the PCN Pharmacy and wider team to ensure collaborative working.
  • Proactively develop relationships with the local ICB medicines management and Community Pharmacy teams.

What You Will Need:

  • A degree in Pharmacy and registration with the General Pharmaceutical Council (GPhC).
  • Minimum of 2 years postgraduate experience in pharmacy, with some experience working in general practice.
  • Member of the Royal Pharmaceutical Society (RPS).
  • Hold or be working towards a postgraduate pharmacy qualification.

Salary: £60,000 - £80,000 per annum, depending on experience.
