You will work closely with our care managers to develop and implement personalized care plans, ensuring that each client's unique needs and preferences are met. Your primary responsibilities will include:

  • Providing exceptional care and support to clients in their own homes, adhering to their individual care plans
  • Collaborating with care managers to create and review care plans, guaranteeing they meet evolving client needs
  • Supervising and mentoring a team of care workers, providing guidance and training as required
  • Conducting regular assessments of clients' needs and adjusting their care plans accordingly
  • Liaising with healthcare professionals and family members to ensure seamless care delivery
  • Administering medication as prescribed and maintaining accurate records
  • Ensuring all care workers adhere to company policies and procedures, upholding regulatory requirements
  • Participating in on-call duties when necessary
  • Maintaining precise and up-to-date client records and reports
  • Continuously striving to enhance the quality of care provided, identifying areas for improvement and implementing solutions
  • Serving as a positive role model for care workers, promoting a person-centered approach to care
  • Assisting with recruitment and training of new care workers as needed
  • Contributing to team meetings and helping shape the overall development of our service
  • Providing out-of-hours coordination support when necessary

Requirements:

To excel in this role, you should possess:

  • NVQ Level 3 in Health and Social Care or equivalent qualification
  • A minimum of two years' experience in a domiciliary care setting
