Key Responsibilities- Provide flexible and responsive support to individuals, prioritizing their needs and goals.
- Work collaboratively with healthcare professionals, social workers, and other team members to ensure comprehensive and coordinated care.
- Assist and enable individuals with activities of daily living, including bathing, dressing, meal preparation, medication management, and mobility support.
- Monitor and evaluate individuals' progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintain accurate and thorough documentation of interactions, care plans, and progress.
- Previous experience in a similar care setting, preferably within a healthcare or social care environment.

- Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
