We are seeking an experienced and passionate Senior Mental Health Practitioner to take on a leadership role within our CAMHS service. The successful candidate will work specifically with children and young people in care and placed out of the Wakefield district.
Key Responsibilities- Liaise with out-of-area CAMHS services to ensure seamless care for our young people.
- Complete desktop reviews of information and assessments to determine the best intervention for each young person.
- Conduct face-to-face therapeutic needs assessments to identify individualized support plans.
- Support placements to prevent breakdown and ensure the best possible outcomes for our young people.
- Work collaboratively with independent therapeutic services to provide comprehensive care.
- Maintain an overview of young people through the Complex Care Panel.
- Consult, liaise, and network with independent fostering agencies and supervising social workers.
- Liaise and support with discharge planning to ensure a smooth transition for our young people.
- Support The Croft Children's Home in providing high-quality care and services.
- Offer training to professionals and foster carers to enhance their knowledge and skills.
- Excellent verbal and communication skills.
- Ability to manage own caseload and work autonomously.
- Excellent negotiation and problem-solving skills.
- Customer service orientation.
- Excellent record-keeping and report-writing skills.
- IT skills.
- Able to work under pressure and make decisions in crisis situations.
