About the Role

We are seeking an experienced and passionate Senior Mental Health Practitioner to take on a leadership role within our CAMHS service. The successful candidate will work specifically with children and young people in care and placed out of the Wakefield district.

Key Responsibilities
  • Liaise with out-of-area CAMHS services to ensure seamless care for our young people.
  • Complete desktop reviews of information and assessments to determine the best intervention for each young person.
  • Conduct face-to-face therapeutic needs assessments to identify individualized support plans.
  • Support placements to prevent breakdown and ensure the best possible outcomes for our young people.
  • Work collaboratively with independent therapeutic services to provide comprehensive care.
  • Maintain an overview of young people through the Complex Care Panel.
  • Consult, liaise, and network with independent fostering agencies and supervising social workers.
  • Liaise and support with discharge planning to ensure a smooth transition for our young people.
  • Support The Croft Children's Home in providing high-quality care and services.
  • Offer training to professionals and foster carers to enhance their knowledge and skills.

Requirements

Desirable Qualifications
  • Relevant professional qualification, nursing, or other professional allied to medicine.
  • Post-qualification training in identified portfolio, LD, LAC, eating disorder.
  • Dip SW or relevant mental health profession.
  • A teaching and assessing qualification.
  • Membership of a professional body.
