About the Role

Oxleas NHS Foundation Trust is seeking a highly motivated and skilled Community Health Nurse to join our Adult Community Health Services team in Bexley and Greenwich District Nursing Teams.

The successful candidate will deliver high-quality care to patients in their home environment, including phlebotomy, wound care, catheter care, and end-of-life care. As a key member of our team, you will be responsible for performing comprehensive assessments and risk assessments, developing and implementing individualized care plans, and monitoring the physical health of patients regularly.

Main Responsibilities
  • Deliver high-quality care in the community
  • Perform comprehensive assessments and risk assessments
