Job Overview

We are seeking an experienced Pharmacy Medication Optimisation Specialist to join our team at Wessex LMCs Ltd. As a key member of our multidisciplinary team, you will play a vital role in ensuring the safe and effective use of medicines within our primary health care services.

Key Responsibilities:
  • Provide expert advice on medication optimisation to doctors, nurses, and other healthcare professionals.
  • Conduct regular medication reconciliations for newly registered patients and following hospital discharge or outpatient consultations.
  • Support the development and implementation of repeat prescribing systems and processes to ensure quality improvement and safe prescribing practices.
  • Coordinate monthly prescription services for care homes.
  • Collaborate with the Prescription Clerks, Patient Services, and Call Hub team to resolve queries relating to prescription requests and improve efficiency and safety in the prescription issuing process.
  • Deal with patient medication queries via telephone call backs, addressing issues such as dose schedules, side effects, adverse effects, and out-of-stock medications.
  • Resolve queries and issue certain medications in line with practice protocols.
  • Participate in practice clinical meetings, patient participation groups, and other relevant meetings to enhance engagement of pharmacy technicians within the GP model and promote issues related to prescribing and medication optimisation.

Educational Requirements:

  • Degree in Pharmacy or equivalent qualification.
  • Registration with the General Pharmaceutical Council (GPhC).

Estimated Salary: £30,000 - £40,000 per annum depending on experience.
