Social Care Professional

We are seeking a skilled Social Care Professional to join our Adult Services team at Ackerman Pierce Ltd. The successful candidate will be responsible for providing high-quality support to adults with varied needs, working closely with our Community Hub Teams, Learning Disability Team, Flow and Capacity Team, Assurance and Safeguarding Team, Deprivation of Liberty Safeguards Team, and Discharge and Review Team.
Key Responsibilities:
- Providing person-centered support to adults with complex needs
- Working collaboratively with our teams to ensure seamless service delivery
- Developing and maintaining strong relationships with service users, families, and carers
- Conducting assessments and creating personalized care plans
- Monitoring and evaluating service user outcomes
Requirements:
- Qualified Social Worker with a passion for adult social care
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong analytical and problem-solving skills
- Commitment to delivering high-quality services that meet the needs of our service users
