Lead Care Provider

We are seeking an experienced care professional to join our team as a Lead Care Provider. In this role, you will be responsible for coordinating care services and leading a team of care providers to deliver exceptional care to our clients.

Responsibilities include:

  • Developing and implementing individualized care plans for clients
  • Leading and managing a team of care providers to ensure high standards of care are met
  • Collaborating with healthcare professionals and families to ensure seamless care coordination
  • Maintaining accurate records of client care plans and progress
Requirements

To succeed in this role, you will need:

  • A degree in Health & Social Care or a related field
  • Previous experience in care management, preferably in domiciliary care
  • Excellent leadership and communication skills to motivate and guide a team
  • Knowledge of healthcare regulations and best practices to ensure compliance and quality care
