We are seeking a skilled and compassionate Healthcare Assistant to join our Specialist Coronary Heart Disease & Heart Failure Team at County Durham & Darlington NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care and support to patients with cardiac conditions.
Key Responsibilities- Provide care and support to patients with coronary heart disease and heart failure, including recording observations and administering medication.
- Work closely with the Heart Failure Specialist Nurses team to deliver care and support to patients in their own homes and in clinic/community venues.
- Deliver cardiac rehabilitation programs to patients with coronary heart disease and heart failure, alongside exercise facilitators and cardiovascular nurses.
- Travel independently around the Derwentside area to provide care and support to patients.
- Experience of working within a healthcare environment, preferably in a community setting.
- Ability to travel independently and safely move and handle loads.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Good organisational and problem-solving skills, with the ability to prioritise work and meet service demands.
- Competent in performing and recording clinical observations, including BP, heart rate, and rhythm, pulse, oximetry, and respiratory rate.
- Ability to observe and record physical and social changes of a patient and report immediately.
- Phlebotomy skills or willingness to work towards.
