Nurse Care Specialist

At Smartcall Healthcare Ltd, we are seeking a highly skilled and compassionate Registered Nurse to join our team in a Care Home setting. As a Care Home Nurse Specialist, you will be responsible for delivering high-quality patient-centred care to our residents, working closely with our multidisciplinary team to ensure the best possible outcomes.
Key Responsibilities:
- Deliver high-quality patient-centred care to residents in a Care Home setting
- Work closely with the multidisciplinary team to ensure the best possible outcomes
- Develop and implement individualised care plans to meet the needs of residents
- Conduct regular assessments and reviews to ensure the quality of care
- Collaborate with other healthcare professionals to ensure seamless care transitions
Requirements:
- Active NMC registration as a Registered Nurse (RGN, RMN, RNLD)
- Experience in a Care Home setting, preferably with a focus on elderly care
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Commitment to adhering to NMC guidelines and code of conduct
