Key Responsibilities:
- Lead and manage a team of support workers to deliver person-centered care and support services.
- Develop and implement care plans that meet the individual needs of our service users.
- Collaborate with other professionals to ensure seamless care and support services.
- Monitor and evaluate the quality of care and support services, making recommendations for improvement.
Requirements:
- Strong leadership and management skills.
- Experience of working with adults with Autism Spectrum Disorder and Learning Disabilities.
- Ability to develop and implement care plans that meet individual needs.
- Excellent communication and interpersonal skills.
