Job Overview

We are seeking a highly motivated and experienced Physiotherapist to join our Home First and Community Therapy Service team. As a Band 6 Physiotherapist, you will play a key role in providing high-quality Physiotherapy interventions to patients in their own homes, promoting independence and well-being.

Key Responsibilities
  • Conduct comprehensive Physiotherapy assessments and develop personalized treatment plans to meet the needs of patients.
  • Work collaboratively with patients, families, and other healthcare professionals to ensure seamless care and effective communication.
  • Take clinical responsibility for a designated caseload of patients, prioritizing care and managing workload effectively.
  • Contribute to the development and implementation of Physiotherapy services, ensuring alignment with Trust priorities and standards.
  • Participate in audit, quality improvement, and research initiatives to drive excellence in Physiotherapy practice.
