Physiotherapist - Community Health Specialist

We are seeking a highly motivated and experienced Physiotherapist to join our Home First and Community Therapy Service team. As a Band 6 Physiotherapist, you will play a key role in providing high-quality Physiotherapy interventions to patients in their own homes, promoting independence and well-being.
Key Responsibilities- Conduct comprehensive Physiotherapy assessments and develop personalized treatment plans to meet the needs of patients.
- Work collaboratively with patients, families, and other healthcare professionals to ensure seamless care and effective communication.
- Take clinical responsibility for a designated caseload, prioritizing tasks and managing workload effectively.
- Contribute to the development and implementation of Physiotherapy services, including audit, quality improvement, and research initiatives.
- Maintain accurate and up-to-date clinical records, adhering to Trust policies and procedures.
