We are seeking an experienced Community Nurse to join our team at Nurslink. As a Community Nurse, you will be responsible for delivering high-quality patient care in a community setting.
Key Responsibilities:
- Carry out care plans in collaboration with patients, families, and multidisciplinary teams.
- Provide skilled nursing care including wound care, medication administration, and symptom management.
- Co-ordinate care and liaise with other healthcare professionals, community services.
- Maintain up-to-date documentation of assessments, care plans, and interventions.
- Excellent clinical assessment and communication skills.
Requirements:
- Band 5 or above.
- Must have a valid driving licence and access to a vehicle.
- Registered nurse with a current NMC pin.
- Relevant Nursing qualifications.
- Hardworking RGN/RMN.
- 6 months recent experience working as a Community nurse.
- 2 clinical references that cover 3 years work experience.
- Valid DBS certificate.
