Job Title: Community Healthcare Assistant - Bank

We are seeking a skilled and dedicated Community Healthcare Assistant to join our team on a temporary basis. As a Community Healthcare Assistant, you will play a vital role in providing high-quality patient care and support in the community.

Key Responsibilities:
  • Provide holistic care to patients in the community, including in their homes, health centers, and residential homes.
  • Work collaboratively with qualified nursing and therapy staff to implement care and rehabilitation programs.
  • Support patients in regaining confidence in maintaining activities of daily living.
  • Ensure effective risk management at team level by reporting accidents and incidents, assessing and controlling risk, and adding residual risks to the Trust register.
  • Provide accurate records of information required by the Trust for audit purposes.
Requirements:
  • NVQ level 2 or equivalent standard of literacy and numeracy.
  • NHS Care Certificate or 2nd year or above student nurse.
  • Ability to work under own direction and under pressure.
  • Ability to work in isolation and as part of a team.
