Community Night Nurse

We are seeking a skilled and dedicated Community Night Nurse to join our Night Nursing (Out of Hours) Team in East Lancashire. As a key member of our team, you will provide high-quality nursing care to patients in the community setting, working across East Lancashire overnight to support our 24-hour nursing service.
Key Responsibilities- Contribute to the assessment, planning, implementation, and evaluation of care for patients in the community setting.
- Work effectively in a multi-disciplinary team to provide holistic care to patients with varying healthcare needs.
- Provide wound management, health promotion, medicines management, and end-of-life care to patients.
- Be able to work contracted hours rotationally across day and night shifts, as reasonably required by the needs of the service.
