**Job Title:** Community Care Specialist
">We are seeking an experienced and motivated individual to join our Integrated Urgent Care Team as a Technical Care Instructor. The successful candidate will work in a community setting, providing safe care to patients within their own homes.
">The main purpose of this role is to ensure that high-quality patient care is provided in a community environment. The post holder will work closely with qualified and non-qualified nursing, therapy, and social work staff to coordinate individualized patient care and maintain trust standards in documentation.
">This is an exciting opportunity for someone who is passionate about delivering care and working collaboratively as part of a multidisciplinary team. If you have experience of working in a community care environment and possess excellent communication skills, we would like to hear from you.
