Job Overview

We are seeking a skilled Community Mental Health Practitioner to join our team in the Peterborough South Adult Locality Team. The successful candidate will work on a full-time basis for one year, providing specialist advice, assessments, and care to a designated caseload.

Main Duties

  • Undertake assessments of individuals and their families in the designated care group with complex health presentations.
  • Develop care plans in collaboration with service users and their families, adhering to recovery principles.
  • Offer specialist assessment and advice where required.
  • Manage and regularly review ongoing risks, ensuring collaboration with service users and carers.
  • Ensure clinical practice is evidence-based and consistent with relevant NICE guidelines and Trust policies.
  • Provide supervision to junior staff and trainees, where appropriate.
