Main Job Responsibilities

The successful candidate will be responsible for delivering high-quality nursing services to patients in a busy primary care setting. This will involve working closely with the multidisciplinary team to ensure the effective delivery of patient care.

The post holder will be responsible for health promotion, chronic disease management, health prevention, and supporting the practice team in the delivery of clinical policy and procedure.

About the Role

This is an exciting opportunity for an experienced practice nurse to join our team at Eastgate Medical Group. We are looking for a highly motivated and compassionate individual who is passionate about delivering high-quality patient care.

The successful candidate will have experience of working in a primary care environment, with a strong background in chronic disease management. They will also have excellent communication and interpersonal skills, with the ability to work effectively as part of a team.

Key Duties and Responsibilities
  • Deliver high-quality nursing services to patients in a busy primary care setting
  • Work closely with the multidisciplinary team to ensure the effective delivery of patient care
  • Develop and implement health promotion and wellbeing programmes
  • Manage chronic disease clinics, delivering patient care as necessary and referring patients to secondary/specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure compliance with the practice chaperone policy
  • Support GPs with minor surgery when required
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
Requirements
  • Registered Nurse (NMC)
  • Post graduate diploma or degree (Chronic Disease Management)
  • Experience of working in a primary care environment
  • Experience of chronic disease management
  • Experience of working as a practice nurse
  • Experience of working with SystmOne (Clinical System)
