Job Overview


We are seeking a dedicated and enthusiastic Healthcare Professional to join our Community Services team within the NHS Foundation Trust. The successful candidate will work alongside District Nurses, registered Community Nurses, Health Care Assistants, and Phlebotomists to provide high-quality nursing care to patients in the community.


This is an exciting opportunity for a Nursing Associate or Assistant Practitioner to develop their skills and experience in a dynamic and supportive environment. The role will involve unsupervised delegated nursing duties, developing effective channels of communication with other professionals and agencies, and participating in audits and service improvement initiatives.


Main Duties of the Job


The successful candidate will be responsible for providing holistic and person-centered care and support for people in their care. They will work within their scope of practice as governed by the NMC Code (2018) and NMC Standards of Proficiency (2018). Additionally, they will supervise and support HCAs, Trainee Nursing Associates, and Pre-registration nurses.

Person Specification


For this role, we are looking for candidates with a Foundation Degree in Health, evidence of further study or development, and at least 2 years of experience in the NHS. The successful candidate will also hold a current driving license.
