This role offers an exciting opportunity for an experienced healthcare professional to join our team as an operational lead. The successful candidate will be responsible for leading a team of healthcare professionals to deliver high-quality care to patients in our community and primary care settings.

Job Summary:

  • Lead a team of healthcare professionals to deliver high-quality care to patients in community and primary care settings.
  • Develop and implement strategies to improve patient outcomes and reduce avoidable hospital admissions.
  • Work closely with other healthcare organizations and stakeholders to ensure seamless care pathways for patients.
  • Provide leadership and guidance to the team, ensuring they have the necessary resources and support to deliver high-quality care.
Required Skills and Qualifications

Essential:

  • Registered Practitioner with current registration.
  • Evidence of relevant post-graduate courses/continual professional development.
  • Significant experience of working within community and primary care settings.
  • Experience of leadership and managing a team.
  • Ability to give clear and concise reports against performance targets.

Desirable:

  • Relevant experience at Band 7.
  • Experience of developing new services.
