Key Responsibilities- Support Transition Planning: Collaborate with the Transition Lead and location-based transition teams to develop and implement transition plans for young people.
- Provide Emotional Support: Offer emotional support and guidance to young people and their families throughout the transition process.
- Build Partnerships: Develop and maintain relationships with external agencies, schools, and care providers to ensure seamless transition for young people.
- Advocate for Young People: Champion the rights and needs of young people with life-limiting conditions, ensuring they receive fair access to care, education, and social activities.
- Experience Working with Children and Young People: Significant experience of working with children and young people with complex needs.
- Understanding of Transition Challenges: Understanding of the challenges faced by young people transitioning into adult care.
- Excellent Communication Skills: Ability to engage well with children, young people, and their families.
- Motivated and Proactive: Motivated to make and sustain positive relationships with external organisations.
