Key Responsibilities- Conduct non-complex assessments for clients, supporting discharge from hospital and preventing admission to hospital.
- Deliver targeted and intensive rehabilitation programs to maximize patient independence.
- Manage a caseload to provide assessment, rehabilitation, onward referral, and discharge to a range of patients referred to the service.
- Collaborate with multi-professional teams to ensure coordinated care for patients.
- Professional guidance and day-to-day management will be provided through an Advanced Practice Occupational Therapist.
- Regular supervision, including peer supervision opportunities, will be offered.
- Opportunities for ongoing professional development will be encouraged.
- A full current driving license and use of a vehicle are essential.
