About the Role

We are seeking an experienced Senior Nurse to join our Early Intervention Community Team within Birmingham Community Health Care NHS Foundation Trust. As a key member of our team, you will work in a rehabilitation setting, providing comprehensive assessments and care plans for patients in their own homes.

With a strong focus on community care, you will work collaboratively with patients, families, and other healthcare professionals to deliver high-quality, patient-centered care. Your expertise will be essential in promoting health, preventing avoidable hospital admissions, and facilitating early hospital discharge.

This is an exciting opportunity to develop your nursing skills and contribute to the success of our Early Intervention programme, which has delivered significant cost savings and improved health outcomes for our patients.

Key Responsibilities
  • Conduct comprehensive assessments of patient care needs and develop individualized care plans.
  • Work collaboratively with patients, families, and other healthcare professionals to deliver high-quality, patient-centered care.
  • Provide education and support to patients and families to promote health and prevent avoidable hospital admissions.
  • Contribute to the development and implementation of quality initiatives, such as falls prevention.
Requirements
  • Registered Nurse Level 1 (Adult) qualification.
  • Significant experience as a registered nurse in a rehabilitation setting or care of the older adult.
  • Experience working in a community setting and knowledge of current legislation affecting community services.
  • Ability to work as a team player and contribute to the success of our Early Intervention programme.
