About the Role

We are seeking a highly motivated and experienced Adult Mental Health Practitioner to join our team at Gloucestershire Health and Care NHS Foundation Trust. As a key member of our Crisis Resolution and Home Treatment Team, you will play a vital role in providing high-quality care to individuals experiencing mental health crises.

Key Responsibilities
  • Provide expert mental health assessment and intervention to individuals in crisis
  • Develop and implement personalized care plans to support service users
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Conduct risk assessments and develop strategies to mitigate risks
  • Provide education and support to service users and their families
Requirements
  • Registered Nurse Mental Health (RMN) or Registered Social Worker (HCPC)
  • Evidence of continued professional development
  • Working knowledge of Early Intervention, Primary Care, Recovery, Crisis Resolution & Home Treatment (CRHT) approach and social inclusion
  • Excellent communication and interpersonal skills
