Complex Mental Health Support Worker

At CJP Outreach Services, we are seeking a highly skilled and compassionate Complex Mental Health Support Worker to join our dynamic team. The successful candidate will provide person-centred support to a nonbinary individual with complex trauma history, enabling them to develop essential life skills and independence.

The ideal candidate will have a background in supporting individuals with personality disorders, complex post-traumatic stress disorder, complex trauma, and autism. Experience in community-based settings, such as supported living and/or residential care, is essential. Knowledge of trauma-informed care practice and Positive Behaviour Support (PBS) is also highly desirable.

As a key team player, the successful candidate will be responsible for:

  • Providing trauma-informed, person-centred support to the individual.
  • Supporting the individual to develop self-management tools to aid in their recovery and live a fulfilled life.
  • Collaborating with the individual to create and implement tailored support plans.
  • Assisting the individual to develop essential life skills, such as cooking, personal hygiene, budgeting, and household management.
  • Supporting with medication, medical appointments, and maintaining good quality health and well-being.
  • Supporting with community activities and enabling the individual to engage in social events, access local services, and build meaningful relationships.
  • Maintaining accurate and comprehensive care records, documenting progress, incidents, and outcomes as required.

The successful candidate will work closely with the individual, their family, external professionals, and other agencies to ensure holistic care. This is a 24/7 role, with a staffing ratio of 2:1 support, and the successful candidate will be expected to work rotational shifts, including every other weekend and sleep-in duties.
