About the Role

South Reading and Shinfield Group Medical Practice is seeking a highly skilled Pharmacy Technician to join its team. As a Pharmacy Technician, you will play a vital role in improving the quality and safety of care delivered to patients across the practice.

Key Responsibilities

• Conduct regular medication audits to ensure key performance indicators are met and consistent practice in GP prescribing is maintained.
• Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.
• Work closely with local pharmacies to improve efficiencies and patient care.
• Ensure dose optimisation on medicines and oversee team work for medication switches and synchronisation.
• Liaise with community pharmacies on the introduction of electronic repeat dispensing.
• Support practice pharmacists and clinicians with medicines reconciliation and update patient medical records and prescriptions.
• Provide expertise to address public health and social care needs of patients, including lifestyle advice and help in tackling local health inequalities.
