Job Summary

Band 6 Community Sister: Hambleton and Richmondshire District Nursing

We are seeking a Registered Nurse with a Specialist Practitioner in District Nursing qualification to fill a Community Sister position aligned with Hambleton North Primary Care Network (PCN) based at Mowbray House Surgery in Northallerton.

The successful candidate will lead and manage a community nursing team, working in a multi-professional environment and contributing to the delivery of high-quality patient care.

Main Responsibilities

Key Duties:

  • Provide exceptional clinical care and support to patients in the community setting.
  • Lead by example in delivering effective evidence-based care to a defined demographic population with diverse healthcare and social needs.
  • Work collaboratively with GP practices and other partners to support patients with complex and high-acuity needs, preventing unnecessary hospital admissions.
  • Develop and implement innovative practice and strategies to improve patient outcomes and experience.
  • Contribute to the development and implementation of the PCN's strategic plan, aligning with the NHS Long Term Plan.

The successful candidate will be expected to attend the leadership development programme and quality improvement training.

This programme aims to:

  • Develop leadership effectiveness and skills.
  • Equip practitioners with the skills to champion, lead, and complete quality improvement within their role.

After completing the four-and-a-half-day training, you will be able to explore further leadership and improvement training opportunities, including in-house courses and leadership apprenticeships at level 3, 5, and 7.
