We are seeking a highly skilled and motivated Clinical Pharmacist to join our team at FedBucks Ltd. As a Clinical Pharmacist, you will play a key role in providing high-quality patient care and contributing to the development of our clinical pharmacy team.
Key Responsibilities- Work collaboratively with a multidisciplinary team to clinically assess and treat patients, utilizing your expertise in medicines management.
- Take ownership of chronic disease management and undertake structured medication reviews to proactively manage patients with complex poly-pharmacy.
- Develop and manage medicines management plans, delivering patient services in line with Network policy and local and national guidance.
- Maximize cost-effective prescribing and improve the quality of patient care.
- Provide specialist expertise in medicines use, addressing public health and social care needs of patients in our PCN practices and tackling inequalities.
- Develop relationships and work closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system.
- Take a central role in clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, and community pharmacists.
We are looking for a candidate with:
- Capacity to innovate and develop the role of a practice pharmacist.
- Effective interpersonal, communication, presentation, and influencing skills.
- Ability to work with a range of clinical and non-clinical personnel as part of a team.
- Ability to communicate medicines and service-related information to decision makers at all levels.
- Ability to work independently and effectively with a high degree of motivation for long periods.
- Ability to prioritize and work to deadlines, often with frequent interruptions and urgent requests.
- Ability to motivate people and facilitate change.
- Ability to define, collate, analyze, and interpret data.
- Able to utilize databases and information technology effectively.
- Ability to communicate information to patients and carers in an appropriate manner.
To be successful in this role, you will need:
- Masters Degree in Pharmacy (MPharm) or equivalent.
- Registration with the General Pharmaceutical Council (GPhC).
- Membership of the Royal Pharmaceutical Society (RPS).
- Specialist knowledge through a Postgraduate Diploma and qualifications (e.g., clinical, community, therapeutics) or equivalent.
- Evidence of recent and relevant Continuing Professional Development.
- At least 2 years post-registration experience in a hospital, community, or general practice setting.
- Experience of undertaking medication or medicine use reviews, patient counselling, and multidisciplinary/multiagency work.
- Experience of facilitating change to improve clinical practice and delivering training sessions to varied groups.
- Experience in undertaking clinical audit.

- Independent prescriber status or a commitment to undertake the course.
