Clinical Pharmacist

FedBucks Ltd is seeking a highly skilled Clinical Pharmacist to join our Dashwood Primary Care Network team. As a Clinical Pharmacist, you will play a vital role in providing high-quality patient care and contributing to the growth and development of our clinical pharmacy team.
Key Responsibilities- Work collaboratively with a multidisciplinary team to clinically assess and treat patients, utilizing your expertise in medicines management.
- Take ownership of chronic disease management and undertake structured medication reviews to proactively manage patients with complex poly-pharmacy.
- Develop and manage medicines management plans, delivering patient services in line with Network policy and local/national guidance.
- Maximize cost-effective prescribing and improve patient outcomes through the effective use of medicines.
- Provide specialist expertise in medicines use, addressing public health and social care needs of patients in our PCN practices and tackling inequalities.
- Develop relationships and work closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system.
- Take a central role in clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, and community pharmacists.
