About the Role

This Community Crisis Specialist position at Lincolnshire Partnership NHS Foundation Trust (LPFT NHS) requires a skilled professional to coordinate care needs, conduct assessments, and develop care plans for individuals with mental health needs.

The estimated salary for this role is £40,800 per annum, plus benefits.

You will work closely with the multi-disciplinary team to ensure safe and effective care delivery. Your expertise will help create a comprehensive care plan, addressing the unique needs of each individual.

Key Responsibilities
  • Develop and maintain accurate and timely clinical records and risk assessments.
  • Co-ordinate and monitor the work of the team to ensure efficient delivery of services.
  • Foster empowering leadership skills within the Crisis Team and promote the profession locally and nationally.
