Job Overview

We are seeking a skilled and compassionate Healthcare Support Worker to join our Psychiatric Liaison team at Ysbyty Glan Clwyd Hospital. As a key member of our multidisciplinary team, you will play a vital role in supporting the liaison team nurses in assessing and risk assessing service users presenting to the emergency department and general/medical wards.

Main Duties
  1. Attend the general hospital wards to complete reviews on patients open to liaison and provide support to the one-to-one mental health needs of some individuals.
  2. Contribute to the organisation and delivery of patient care and service delivery across a large geographical area in YGC.
  3. Develop innovative and effective approaches to patient care and service delivery, working collaboratively with other experienced nurses and healthcare professionals.
  4. Communicate effectively with colleagues and patients, demonstrating strong team working skills and a commitment to shared responsibility.
  5. Support the vision and ability to help move the service forward, while getting the best out of colleagues and team.
