Dementia Nurse Practitioner Role in Innovative Community

In this role, you will be responsible for delivering person-centred dementia care, working collaboratively within a multidisciplinary team to provide holistic nursing assessments and general nursing and bio-psychosocial interventions.

The ideal candidate will possess:

  • An active NMC Pin
  • Minimum of 2 years' practice experience in a dementia care community
  • Evidence of delivering person-centred care for people with complex dementia needs
  • Commitment to quality care and professional development
