Join the Wigan Mental Health Liaison All Age Service as a skilled practitioner, providing specialist mental health screening and comprehensive biopsychosocial assessment to individuals of all ages. The successful candidate will formulate care plans, conduct risk assessments, and signpost or discharge to appropriate services, working as part of an integrated multidisciplinary team.

Main Responsibilities

  • Conduct comprehensive biopsychosocial assessments to identify individuals' mental health needs and develop care plans.
  • Provide specialist mental health screening and risk assessments to individuals attending the Accident and Emergency department, including the Paediatric Unit.
  • Work collaboratively with other services to ensure timely discharge and improved quality of life for patients referred to the service.
  • Represent mental health services within an acute hospital setting, promoting joint working and collaboration across the Urgent Care Pathway.
  • Work with families and carers to promote wellbeing and support individuals in their recovery.

Person Specification

  • Registered RMN, Social Work registered or HCPC.
  • Experience of mentorship, supervision, and coordination of staff.
  • Experience of care planning and risk assessment.
  • Knowledge of mental health legislation and policies.
  • Ability to work effectively in a multidisciplinary team.
