Patient Care Coordinator Job Description

The Limes Medical Centre seeks a compassionate and detail-oriented Patient Care Coordinator to join their team at Finch Road Primary Care Centre. As a key member of the healthcare team, you will play a vital role in coordinating patient care, conducting health checks, and maintaining accurate records.

Key Responsibilities:

* Coordinate patient care and communicate effectively with healthcare professionals
* Conduct health checks, screenings, and assessments as required
* Develop and implement individualized care plans
* Maintain accurate and up-to-date patient records
* Run clinics and collaborate with nurses to ensure seamless patient care

Requirements:

* 2+ years' experience as a Healthcare Assistant in General Practice
* NVQ in Health and Social Care (L3) and GCSEs in English and Maths
* Strong organizational and communication skills
* Ability to work effectively in a team environment
* Estimated salary: £25,500 - £30,000 per annum
