Community Triage Clinician

About the Role
We are seeking a skilled and compassionate Community Triage Clinician to join our team in the North Locality Hub. As a key member of our triage team, you will play a vital role in assessing patient needs, determining priority of care, and processing referrals accordingly.
Key Responsibilities
- Process referrals by evaluating information received and allocating to appropriate services, indicating priority response necessary.
- Document triage notes to ensure patients' records are complete and for audit purposes.
- Liaise and communicate with the general public, other healthcare professionals, and multi-agencies on a daily basis.
- Assess, prescribe, and deliver care packages for patients with long-term conditions, palliative care, and rehabilitation needs.
- Work within the integrated team to facilitate early discharge from hospital and prevent unnecessary admission.
- Collaborate with healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
Requirements
- Diploma or 1st-level degree, or equivalent knowledge and experience.
- Current NMC registration.
- ENB998/mentorship preparation or equivalent.
- Registered General Nurse Qualification.
Desirable Qualities
- Knowledge of additional specialist areas through in-house training and short courses.
