Job Title: District Nurse Team Manager

We are seeking a skilled and experienced District Nurse Team Manager to join our team at Hounslow and Richmond Community NHS Healthcare Trust. As a key member of our community nursing team, you will be responsible for managing a caseload of patients in Hounslow, providing clinical leadership and management to the team, and working in partnership with primary health care teams, social services, and other stakeholders to deliver high-quality patient care.

Main Responsibilities:
  • Provide clinical leadership and management to the Hounslow Community Nursing team
  • Manage a caseload of patients in Hounslow, including case management of patients at risk of hospital admissions
  • Work in partnership with primary health care teams, social services, and other stakeholders to deliver high-quality patient care
  • Develop and implement effective care plans to meet the needs of patients
  • Lead by example, demonstrating a commitment to excellence in patient care and team management

Requirements:
  • Qualified RGN with current registration with NMC
  • Diploma or BSc (Hons) Community Health (DN)
  • Relevant post-registration study at diploma/degree level and relevant CPD
  • Nurse Prescriber
  • Physical Assessment Skills
