Job Title: District Nurse Team Manager

We are seeking a skilled and experienced District Nurse Team Manager to join our team at Hounslow and Richmond Community NHS Healthcare Trust. As a key member of our community nursing team, you will be responsible for managing a caseload of patients in Hounslow, providing clinical leadership and management to ensure the delivery of high-quality care.

Main Responsibilities:
  • Provide clinical leadership and management to the Hounslow Community Nursing team to meet the needs of the caseload and local population.
  • Work in partnership with the primary health care team, social services, and the wider multidisciplinary locality team to provide holistic assessment of health needs and provision of nursing care to housebound people with a range of medical needs and long-term conditions.
  • Manage a caseload of patients in Hounslow, including case management of patients identified as being at risk of hospital admissions.

Requirements:
  • Qualified RGN with current registration with NMC.
  • Diploma or BSc (Hons) Community Health (DN).
  • Relevant post-registration study at diploma/degree level and relevant CPD, e.g., palliative care, tissue viability.
  • Nurse Prescriber.
  • Physical Assessment Skills.
Skills and Abilities:
  • Holistic Assessment skills.
  • Core clinical skills.
  • Ability to work independently and as part of a multi-disciplinary team.
  • Ability to lead by example, both clinically and managerially, being consistent and fair to all staff in every aspect of work.
  • Conflict management and mediation skills.
  • Able to demonstrate understanding of Safeguarding Adults & Children.
  • Good knowledge of NMC Guidelines and Standards.
  • Knowledge of policies, professional standards, guidelines, and protocols.
Experience:
  • Experience in assessing, planning, delivering, and evaluating patients in their own homes.
  • Experience in working independently and as part of a team.
  • Experience in problem-solving and supporting other team members.
  • Experience of Multidisciplinary team work.
  • Experience of mentoring students.
  • Experience of managing resources to stay within the service budget.

  • Flexible.

  • Car Driver or Competent Cyclist.
  • Access to own transport.
