About the Role

We are seeking a highly skilled and dedicated Registered Nurse to join our Community Nursing Team in Oldham. As a key member of our team, you will be responsible for providing high-quality patient care in a variety of home environments.

Key Responsibilities
  • Deliver patient-centered care to individuals in their own homes, promoting independence and well-being.
  • Work collaboratively with other healthcare professionals to develop and implement effective care plans.
  • Conduct thorough assessments and provide evidence-based interventions to meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in ongoing professional development to stay up-to-date with best practices and advancements in community nursing.
Requirements
  • Current registration with the Nursing and Midwifery Council (NMC).
  • Relevant nursing qualifications and experience in community care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work across different geographical locations within the Trust footprint.
