We are seeking a skilled and compassionate Adult Social Worker to join our Adult Community Team. As a key member of our team, you will play a vital role in supporting adults with complex needs, including physical disabilities, mental health challenges, and learning disabilities.

Key Responsibilities
  • Conduct thorough assessments of individuals' needs, creating personalized care plans that cater to their unique requirements.
  • Facilitate access to essential services that enhance well-being, ensuring compliance with statutory requirements under the Care Act 2014.
  • Participate in safeguarding and care planning processes, working closely with experienced practitioners and multi-disciplinary partners.
  • Carry out mental capacity assessments, making applications for Deprivation of Liberty Safeguards (DoLS) as necessary, while upholding individuals' rights and legal standards.
  • Manage a caseload, conducting robust assessments in partnership with service users, their carers, and relevant stakeholders.
  • Maintain accurate, clear, and timely records of assessments, interventions, and care plans, adhering to council policies and legal standards.
Requirements
  • Relevant social work qualification.
  • At least 2 years of experience in adult social work, including assessment and care planning.
