About the Role

We are seeking a highly motivated and compassionate Support Worker to join our Mental Health & Complex Needs Service at Hestia. As a key member of our team, you will play a vital role in supporting service users to achieve their goals and live independently.

Key Responsibilities
  • Provide person-centred support to service users, using a key-work system to enable them to attain and sustain a tenancy, enjoy a good quality of life, and avoid hospitalisation or relapse.
  • Assess referrals, formulate and review support plans, risk management plans, and move-on plans with service users.
  • Work in partnership with external agencies to ensure comprehensive care and support for service users.
  • Facilitate access to health services, education, training, and employment opportunities.
  • Work flexibly as part of the team to ensure service coverage and meet contractual obligations.
Requirements
  • Experience of working with people with diverse support needs, aspirations, and associated risks.
  • Delivering high-quality support services to individuals with varying needs and risk.
  • Proficiency in using computer-based systems to record work and client details.
  • Knowledge of welfare benefits, housing options, and relevant legislation.
  • Understanding of health and safety protocols relevant to working in service users' homes.
  • Ability to undertake client-focused key working, support planning, and risk assessments.
  • Good understanding of issues, diagnoses, and treatments relevant to the client group, including substance misuse.
  • Ability to liaise with external professionals to inform and develop service users' support packages.
  • Willingness to engage in relevant personal development and training.
  • Ability to travel throughout the Borough of operation.
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately.
